How to sign up to our Clinic?
The declaration of selecting a primary care physician can be submitted in three different ways:
- In person at the Clinic
In order to sign up, patients should go to the registration desk with their ID and fill out the declaration. The form of the declaration is available at the Clinic or on the following website: http://www.nfz.gov.pl/dla-swiadczeniodawcy/pliki-do-pobrania/druki/
When accepting the declaration, the registration desk employee is required to check the applicant’s insurance status via the eWUŚ system (art. 56).
In the event of a negative result of the eWUŚ verification, the patient must submit a written statement (providing their name, address, PESEL number or date of birth, and the number and type of identity document) that they are entitled to healthcare services.
- Via the Online Patient Account
In order to sign up via the Online Patient Account, the patient must have a Trusted Profile or an electronic ID. At https://pacjent.gov.pl/, the patient logs in to his Online Patient Account. After entering the “My Account” section, in the “Doctor” tab, they must choose the “Fill in the declaration” option.
- By electronic means
The patient can provide an application bearing a qualified electronic signature, a Trusted Signature or a personal signature, sent via electronic means of communication.
Important! Due to the ongoing state of pandemic, a scan of a correctly completed and signed declaration of choice of a primary care doctor can be sent by email.
